Please bring completed forms to your first appointment in order to expedite the initial paperwork process and reduce waiting times. Appointments, Changes, and Cancellations
We realize that our patients' time is valuable, and we make every effort to see patients at their scheduled appointment time. We ask that patients provide as much advance notice as possible when running late for an appointment or canceling. With this advance notice of cancellation, the time can be scheduled for another patient.
What to Bring to Your Appointment
If you are a first-time patient, complete a copy of the new patient form you will find above and bring it with you. Please also bring the following items to the appointment:
- Photo identification
- Insurance card
- Any required co-payment
- List of all over-the-counter and prescription medications that you are currently taking
- Pertinent medical and surgical history information
- X-ray films that you may have
We will be more than happy to submit all insurance claims for you to help you take full advantage of your insurance benefits. We encourage all patients to confirm their coverage prior to making an appointment. Please click here for list of accepted insurance plans.
Co-payments for podiatric services are due at the time of the appointment. You are responsible for any unpaid balance not covered by your insurance. If necessary, we can design a payment schedule to help you afford the treatment you need and desire.
Forms of payment accepted include cash, check, Visa and Mastercard. Please call us with any questions regarding which insurance plans we accept, or with any billing concerns. Insurance coverage and benefit questions should be directed to your health insurance provider.
